In this role, you will be responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals. SNBC Care Coordinator Position Description
Department: Community Engagement
Reports To: Manager of Community Care Coordination
Pay Grade: Grade 8: Non-Exempt
Supervises: None
Revision Date: March 2026
JOB PURPOSE or JOB SUMMARY:
This position is fast paced with a high caseload; responsible for completing health assessments in a timely manner, developing and implementing care plans for enrollees, and assisting enrollees in achieving their health goals. This position acts as the care coordinator working with an interdisciplinary care team to manage the care and pharmacotherapy of enrollees.
QUALIFICATIONS:
Required: Current, valid and unrestricted RN license in Minnesota or bachelor’s degree in social work or in the behavioral science field. Minimum of 3 years of clinical experience with at least 2 years of that experience in public health, long-term care, or other settings that cater to vulnerable adult populations, such as experience working directly with the programs that serve frail members and/or vulnerable adults.
Preferred: Two plus years of care coordination experience; county public health or human services contracted care coordination services or other health plan experience.

Performs advanced troubleshooting or analysis; uses judgment to select from a complex set of known alternatives. Work is not checked; impact of errors or negligence would cause waste of time and/or materials or affect enrollee’s ability to access services or relationships with providers.
Relationships:
Maintains satisfactory relationships; frequent interaction to provide detailed procedural explanations or technical responses; may include collaboration in executing established policies, communication with stakeholders (members, providers, county, peers, etc.) is frequent.
Physical Demands
Role primarily involves sedentary work requiring extended periods of sitting, frequent use of computers, phone, and engagement in meetings. The position demands sufficient visual acuity for reviewing detailed regulations and updates, DHS contracts, audit documentation, and other privacy documents, as well as the ability to communicate effectively in person and via digital platforms (computer, phone). The SNBC Care Coordinator must be capable of maintaining focus and accuracy under pressure. Requires high attention to accuracy, precision, details and deadlines

Travel Requirements:
